Once, during a FACS check, I accidentally set off a fire extinguisher in the production area. It was not so funny at the time.

Routing colour bars to every single router output in the entire truck. This was for 9 seconds during the live broadcast of French Premier League football (Soccer) It was a long nine seconds :-)
